During the appointment booking process, you will come across intake forms uploaded by the provider specifically for the service you are booking. Please ensure you download and fill out this form when necessary.

If you are scheduling with a new provider, it is your responsibility to download, complete, and submit all relevant intake forms before your appointment begins. Materia+ does not walk you through the intake form process.

Note: Please make sure to download the appropriate intake form(s) that pertain to your situation. For example, your provider may have both an adult intake form and a separate child intake form available. If the appointment is for you as an adult, please download and fill out the adult intake form. If the appointment is for your child, be sure to download and complete the child intake form.

How do I know which intake form to download?

During the booking process, providers will specify which intake form to download. Typically, intake forms are intended for new patients.
