Learn how to create a patient account on Materia+

Anyone who schedules an appointment with a provider must have a Materia+ account.
You can create an account here or by clicking Join in the navigation menu at the top of Materiaplus.com. Once there, fill in your information to set up your account. A verification email will be sent to you —please follow the instructions provided in that email to finalize the account setup.
If you don’t want to make an account in advance, you’ll be prompted to create one when you attempt to book an appointment.
Please make sure to input your legal first and last name, along with the correct country, postal code, sex, and date of birth. This information allows your provider to create a profile for you and deliver a more tailored service.
Note: Materia+ utilizes a password login system. To ensure your account remains as secure as possible, remember to update your password every few months.
